Job summary

Qualifications

• Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred.
• Requires an active Registered Nurse (RN) license in Indiana or NLC license.
• RN must have graduated from a nationally accredited nursing program.
• Basic Life Support (BLS) certification required.
• Other advanced Life Support certifications may be required per unit/department.
• Ability to assess patients without face-to-face interaction may be required.

Responsibilities

• Provide care coordination, chronic disease management, and patient education.
• Manage medications and coordinate referrals to ensure continuity of care.
• Perform post-hospitalization follow-up for patients across the lifespan.
