Qualifications

• Doctor of Pharmacy degree from an accredited college or school of pharmacy.
• Current pharmacist licensure, or eligibility for licensure, in South Carolina.
• Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent relevant clinical pharmacy experience preferred.
• Experience in ambulatory care, primary care, population health, transitions of care, medication therapy management, or chronic disease management preferred.
• Experience working within an electronic health record and documenting clinical pharmacy interventions preferred.
• Experience with pharmacist collaborative practice models.
• Experience supporting quality improvement, population health, or value‑based care initiatives.
• Experience managing high‑risk or medically complex patient populations.

Responsibilities

• Medication Therapy Management (MTM) and targeted reviews for high‑risk patients.
• Annual Wellness Visits (AWVs) and preventive care coordination.
• Support value‑based care initiatives with pharmacist interventions and quality measures.
• Transitions of care: post‑discharge reconciliation and optimization.
• Identify and resolve medication discrepancies and optimize therapy.
• Collaborate with physicians, nurses, and care teams to improve outcomes.
