Job summary

ChenMed LLC is seeking an Ambulatory Pharmacist to optimize medication therapy in primary care clinics. This patient-centered clinician partners with physicians, nurses and care teams to improve outcomes and appropriate use of medications.

Under a collaborative practice agreement, you will conduct comprehensive med reviews, manage chronic diseases, coordinate transitions of care, educate patients and monitor lab results to ensure safety, efficacy, and cost effectiveness.

Qualifications

  • Doctorate of Pharmacy (PharmD) required.
  • Active pharmacy license in good standing.
  • Ambulatory care residency or BCACP certification preferred.
  • Minimum two years of clinical experience in primary/ambulatory care.

Responsibilities

  • Collaborates with providers to adjust medications and interpret lab results within a protocol-driven framework.
  • Conducts comprehensive medication reviews addressing safety, efficacy, cost, side effects, and adherence barriers.
  • Proactively manages chronic diseases such as diabetes, hypertension, anticoagulation, and asthma.
  • Facilitates transitions of care by resolving med discrepancies during hospital discharges to prevent readmissions.
  • Delivers personalized counseling on medication use, side effects, and adherence.
  • Orders and interprets laboratory results to monitor drug safety and effectiveness.
  • Works with physicians to implement care protocols and independent med adjustments when appropriate.
  • Supports department quality improvement initiatives.

The Ambulatory Pharmacist is a patient-centered clinical professional dedicated to optimizing medication therapy within primary care clinics. Operating under a collaborative practice agreement, this role focuses on reducing polypharmacy, enhancing medication adherence, and managing chronic diseases to improve patient outcomes and elevate quality performance metrics such as CMS Star Ratings and HEDIS scores. The ambulatory pharmacist partners closely with patients, physicians, advanced practice providers, nurses, and care teams to deliver evidence-based, individualized care that promotes safety, efficacy, and cost-effectiveness.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES
  • Medication Management & Optimization: Collaborates with healthcare providers to adjust medication therapies, interpret lab results, and support clinical decision-making within a protocol-driven framework.
  • Comprehensive Medication Reviews: Conducts detailed evaluations of medication regimens addressing safety, efficacy, cost considerations, side effects, and barriers to adherence.
  • Chronic Disease Management: Proactively manages conditions such as diabetes, hypertension, anticoagulation, and asthma to improve control and prevent complications.
  • Care Coordination & Transitions: Facilitates seamless transitions of care by resolving medication discrepancies during hospital discharges and preventing readmissions.
  • Patient Education & Engagement: Delivers personalized counseling on medication use, side effects, and adherence strategies to empower patients and improve compliance.
  • Lab Testing & Monitoring: Orders, interprets, and utilizes laboratory results to monitor medication, safety and effectiveness.
  • Collaborative Practice & Protocols: Works directly with physicians and providers to implement and follow care protocols, including independent medication adjustments when appropriate.
  • Administrative & Support Activities: Assists with physician consultations, provides accurate drug information, and supports the department’s quality improvement initiatives.
  • Performs other duties as assigned and modified at manager’s discretion.
KNOWLEDGE, SKILLS AND ABILITIES
  • Knowledge of and experience with state and federal insurance programs
  • Familiarity with appropriate local, state, and federal pharmacy-related regulations
  • Ability to work effectively both independently and as part of a multidisciplinary team
  • Excellent organizational, communication, and clinical decision-making skills
  • Strong attention to detail and ability to manage multiple priorities
  • Willingness to travel locally up to 50% of the time; flexible to work evening, weekends and/or holidays as needed
  • Fluency in spoken and written English
  • This job requires use and exercise of independent judgment
EDUCATION AND EXPERIENCE CRITERIA
  • Doctorate of Pharmacy (PharmD) required
  • Valid pharmacy license in good standing (Doctor of Pharmacy/PharmD) required
  • Pharmacy residency or ambulatory care certification (e.g. BCACP) preferred
  • Minimum two (2) years of relevant clinical experience in primary care, ambulatory care, managed care, or related setting
  • Certification in immunization and medication therapy management is a plus
  • Residency training or ambulatory care certification is highly preferred
  • Experience in managed care, HMO, PBM, or MSO settings is a plus
PAY RANGE

$118,062 - $168,658 Salary The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for a bonuses or commissions.
