Job summary

U.S. Department of Veterans Affairs seeks an ambulatory care clinical pharmacy specialist (CPS) to plan, implement, and operate assigned clinical pharmacy programs within the pain management team. The CPS serves as the medication expert and consultant to other health care professionals.

The role provides clinical coverage in pain management clinics, performs medication reconciliation, ensures formulary adherence, and supports safe and effective pharmacotherapy across the care team. A Pharm.D.

Qualifications

• U.S. citizenship / admissibility to hire in VA system.
• Full license to practice pharmacy in the United States.
• Education and licensure verified per VA standards.
• English proficiency in speaking and writing.

Responsibilities

• Plan, implement and operate clinical pharmacy programs in pain management.
• Provide medication therapy management and recommendations to clinicians.
• Perform medication reconciliation and safety reviews.
• Coordinate formulary management and drug usage evaluations.
• Supervise and educate staff and students as needed.
