Ambulatory Case Management, within the Health Management and Education department, serves to improve the health of high-risk patients through empowerment, self-management support, care coordination, case management, and education. The program aims to develop an effective and cost-efficient system of delivering and managing patient-centered care with a focus on chronic illness requiring case management.
