Job summary

Bassett Healthcare Network in New York seeks a Licensed Practical Nurse to provide direct and indirect patient care within ambulatory and advanced practice settings. You will collaborate with RNs, NPs, and physicians to deliver high-quality, patient-centered care and may conduct home visits as needed.

The role emphasizes accurate data collection, adherence to practice standards, ongoing credentials, and patient advocacy.

Qualifications

• Graduation from a Practical Nursing program, required
• Graduate Nurse, required
• Previous healthcare experience, preferred
• Current NYS licensure as a Practical Nurse or valid interim permit, required
• BLS certification within 6 months of hire, required
• Additional credentials are determined by specific work unit, required

Responsibilities

• Provides direct and indirect nursing care individualized to the patient's needs
• Collects and records relevant patient health data accurately
• Evaluates health data to identify patient problems
• Implements and documents specified interventions
• Identifies deviations from expected patient outcomes
• Adheres to approved, research-based practice standards
• Directs and supervises patient care provided by other LPNs and support personnel
• Identifies practice problems and seeks solutions through communication
• Performs and documents Point of Care testing and quality control
• Maintains and documents testing reagents/supplies and equipment
• Advocates for patients to ensure care preserves autonomy, dignity and rights
• Delivers care in a non-judgmental and non-discriminatory manner
• Ensures informed consent signed for indicated procedures
• Maintains current licensure, certification, and competencies
• Provides proof of current BLS validation
• Completes required unit credentialing and annual training
• Practices within NYS-defined LPN scope of practice
• Acquires and maintains current knowledge and skills
• Participates in required educational hours annually
• Demonstrates problem-solving ability
• Performs other duties as requested
