Qualifications

• Pharm.D. or BS Pharmacy from an accredited institution.
• Active Ohio pharmacist license.
• Current BLS certification (AHA).
• Immunization administration certification within 90 days of hire.

Responsibilities

• Monitor and manage medication therapy for patients in ambulatory/outpatient settings.
• Collaborate with clinical teams to optimize therapy and safety.
• Educate patients and families on treatment plans and adverse effects.
• Ensure documentation of interventions and regulatory compliance.
