Qualifications

• 3 years of experience in ambulatory clinical nursing settings, specialty practice, and/or provider office.
• Strong telephone triage experience with patient assessment, prioritization, and follow-up communication.
• Experience supporting endocrinology care, diabetes education, and insurance or prior authorization processes preferred.

Responsibilities

• Care coordination for endocrine patients.
• Patient education on diabetes, thyroid conditions, and related therapies.
• Medication management and timely refills.
• Telephone triage and patient follow-up.
• Support of individualized endocrine care plans.
