Job summary

Diagnostic Laboratory Services, Inc. is seeking a clinical pharmacist to deliver comprehensive medication management across the care continuum, including newborn to geriatric patients.

The role covers in-clinic and community-based services, with emphasis on education, safety, and evidence-based pharmacotherapy. The ideal candidate holds a valid Hawaii pharmacist license and a PharmD (or BS in Pharmacy), with residency or board certification preferred.

Qualifications

• Current Hawaii State license as a Pharmacist.
• Doctor of Pharmacy degree or Bachelor of Science in Pharmacy degree.
• Post-graduate residency accredited by the American Society of Health-System Pharmacists preferred.
• Board certification preferred.
• Current certification as a Certified Diabetes Care and Education Specialist (CDCES) preferred.
• Current BLS certification preferred.
• Experience to demonstrate ability to communicate professional expertise to all levels of the health care team.
• Clinical ambulatory care pharmacist experience preferred.

Responsibilities

• Implements, maintains and provides clinical pharmacist services including: clinical, educational and informational functions for newborn, infant, pediatric, adolescent, adult and geriatric patients.
• Visits and provides clinical pharmacist services to patients in the community.
