Job summary

Summit Pacific Medical Center seeks an Ambulatory Clinical Pharmacist to collaborate with the primary care team to optimize medication therapy and safety for patients. This role focuses on ambulatory care, chronic disease management, and collaboration with providers at Summit Pacific Medical Center.

The pharmacist will conduct patient visits, perform medication consults, help develop treatment protocols, and support the anticoagulation clinic. Washington license and PharmD are required.

Qualifications

• PharmD from an accredited school of pharmacy.
• A post-graduate year residency or two years' experience in the ambulatory clinical pharmacy field.
• Active Washington State Department of Health pharmacist's license.

Responsibilities

Job Summary

The Ambulator Clinical Pharmacist will collaborate with the primary care team at Summit Pacific Medical Center (SPMC) to provide medication and chronic disease state management with an emphasis on optimizing medication therapy and safety for patients.

Job Duties and Responsibilities

• Provide medication dosing support for SPMC Anticoagulation Clinic
• Cross-training with SPMC Pharmacy Department

Required Education and Experience

Required Licenses, Certifications and/or Registrations

Active Washington State Department of Health pharmacist's license

Preferred Board-Certified Ambulatory Care Pharmacist (or equivalent)

Current BLS certification.

Ability to work collaboratively in a multidisciplinary team of health care providers

Skilled in-patient communication and education

Skilled in-patient assessment, medication adjustment, monitoring, and all other relevant aspects of patient care.
