Job summary

Shields Health Solutions is seeking an Ambulatory Clinical Pharmacist (TX license required) to deliver patient-centered care in Corpus Christi clinics. On-site role emphasizing chronic disease management, pharmacotherapy optimization, and collaboration with hybrid/onsite teams.

The ideal candidate has a PharmD and Texas license with 3+ years of clinical pharmacy experience, strong communication, and a focus on confidentiality and high-quality care.

Qualifications

• PharmD and active Texas license required.
• Minimum 3 years as a clinical pharmacist; PGY1/PGY2 residency preferred.
• Strong clinical judgement, communication and process management.
• Deep knowledge of pharmacology and FDA regulations.
• Ability to deliver high-quality, patient-centered care with integrity.

Responsibilities

• Identify and resolve medication-related problems for safe pharmacotherapy.
• Assess chronic disease management per clinic protocols.
• Stratify patients by risk to tailor care plans.
• Counsel on medication use, lifestyle changes and education.
• Address barriers to adherence including cost and health literacy.
• Monitor regimens for goals, adverse effects and interactions.
• Collaborate on-site with interdisciplinary teams.
• Ensure compliance with FDA guidance and literature.
