Job summary

Baptist Health is seeking an ambulatory clinical pharmacist to enhance medication therapy disease management for ambulatory patients. This role supports clinics and works with a multidisciplinary clinical team within the Baptist Health Specialty Pharmacy or embedded clinics.

Pharmacists will interact with patients and providers both remotely and in person as required, ensuring safe, effective and cost-conscious care across ambulatory settings.

Job description

Ambulatory clinical pharmacists will practice within the Baptist Health Specialty Pharmacy or embedded within a multidisciplinary clinical team to support clinics. The primary responsibility is to assure the provision of safe, efficacious, cost-effective, and high-quality medication therapy disease management to patients in the ambulatory setting.
