Job summary

Axil Health is seeking a Pharmacist Fellow to support ambulatory care across community clinics. You will manage chronic disease programs, coordinate with physicians, and mentor residents. This role emphasizes hands-on patient care, data-driven outcomes, and collaborative practice within a growing team.

The program welcomes both new graduates and experienced pharmacists seeking fellowship opportunities, with travel to multiple clinics as needed.

Qualifications

• Pharm.D. from an accredited program and active licensure.
• FAPhA accreditation is required or in progress.
• Strong knowledge of chronic disease management.
• Proficiency with EHR and pharmacy management systems.

Responsibilities

• Provide comprehensive medication management for chronic diseases.
• Collaborate with providers to implement individualized treatment plans.
• Utilize remote patient monitoring to track progress and intervene as needed.
• Lead patient education and adherence initiatives with care teams.
• Participate in interdisciplinary rounds and case discussions.
