Qualifications

• Clinical residency completed or equivalent direct patient-care experience.
• Strong knowledge of pharmacology and disease states.
• Excellent verbal and written communication.

Responsibilities

• Promotes rational and cost-effective drug therapy through decisions with physicians and other medical personnel.
• Provides direction to Pharmacist Residents, Interns, Technicians, and others.
• Conducts patient medication history inquiries.
• Synthesizes treatment plans based on diagnoses and transitions care.
• Identifies and resolves medication-related problems.
• Initiates, adjusts, monitors, and recommends changes in drug therapy.
• Coordinates and interprets laboratory monitoring of drug therapy.
• Provides patient monitoring to evaluate outcomes and adherence.
• Provides patient education regarding disease state and medication use.
• Assure ongoing performance improvement of pharmaceutical care services.
• Educates providers and staff via presentations and reviews.
• Performs other duties as required.
