Job summary

The VA Bedford Healthcare System in Bedford, MA is looking for an Outpatient Primary Care LPN/LVN to provide patient-centered nursing care under the supervision of a registered nurse or provider, supporting safe and efficient clinic operations within the PACT team.

Applicants must be a graduate of an approved practical nursing program with an active LPN/LVN license and English proficiency. Collaboration with RNs/MDs is essential to delivering high-quality care in a busy outpatient setting.

Qualifications

• Licensed practical or vocational nurse with active, unrestricted state license.
• Graduate of an approved practical/nursing program.
• English proficiency for direct patient care as required by policy.
• Special waiver provisions for certain military healthcare education may apply.
• Ability to work under supervision within a clinical team.

Responsibilities

• Provide patient-centered nursing care under RN/provider supervision.
• Support safe, efficient clinical operations in ambulatory care.
• Assist with scheduling, patient flow, and pre-visit preparation.
• Educate patients and families on procedures, medications and follow-up.
• Collaborate with the PACT team to ensure continuity of care.
