Job summary

CoxHealth is seeking an Ambulatory Care Coordinator I to join the Population Health team in Missouri. The role supports care teams by coordinating activities across clinical and community settings, focusing on gaps in care and timely patient outreach.

You will schedule appointments, organize resources, and assess social determinants of health while working under indirect supervision. Strong teamwork and communication are essential.

Qualifications

  • Bachelor's Degree in Health/Human Services related field or 8 years' related experience.
  • 1 year of customer service experience required.
  • Preferred: 1 year prior experience with clinic operations.
  • Must be team-oriented with strong leadership, communication, problem-solving, and organizational skills.

Responsibilities

  • Coordinate efforts related to Population Health and related care teams.
  • Communicate with patients regarding open gaps in care (medication adherence, screenings).
  • Schedule appointments and arrange resources (medications, financial assistance, formulary).
  • Assess social determinants of health and coordinate community resources.
  • Collaborate with clinic staff to ensure smooth operations.

Job Description

Ambulatory Care Coordinator I positions assist the care teams of CoxHealth (e.g. Population Health Services) by coordinating efforts related to our specialty and managed care populations. Position works under indirect supervision and works in both clinical and community-based settings. Responsibilities can include, but are not limited to, communicating directly with patients regarding open gaps in care (e.g. medication adherence, breast cancer screenings, colorectal cancer screenings), scheduling appointments, procurement and organization of resources (e.g. medications, financial assistance, ambulatory formulary) and assessing social determinants of health. Must be team-oriented and have strong leadership, communication, problem-solving, and organizational skills.

Education
  • Required: Bachelor's Degree in Health/Human Services related field OR 8 years' related experience
Experience
  • Required: 1 year customer service experience
  • Preferred: One year prior experience with clinic operations
Skills
  • Ability to navigate and properly utilize the electronic medical record, Microsoft Office products (e.g. Excel), and payer provided platforms
  • Detail oriented, with excellent organizational skills and the ability to prioritize reports and data management
  • Ability to manage multiple tasks concurrently
  • Excellent communication skills required, with ability to interact with patients, clinic staff, and team members in a professional and courteous manner
