MemorialCare is seeking a Clinical Pharmacist Per Diem to join the Pharmacy Specialty Clinics in Fountain Valley and Carson, CA. This ambulatory care role focuses on chronic disease management within the ACTIVE Care Clinic, delivering evidence-based medication management and patient counseling.

The ideal candidate has a Doctor of Pharmacy, a post-graduate residency, California Pharmacist License, and at least two years of ambulatory experience.

• Minimum 2 years clinical ambulatory pharmacy experience.

Mansioni

• Provide direct patient care in the ACTIVE Care Clinic.
• Manage chronic diseases such as diabetes, hypertension, anticoagulation, and heart failure using approved protocols.
• Perform comprehensive medication management and reconciliation.
• Educate patients on medications, disease states, lifestyle modifications and adherence strategies.
• Collaborate with physicians, case managers, and other professionals to optimize care.
• Document clinical interventions in the EHR.
• Support quality improvement and clinical workflows.
• Stay current with ambulatory care guidelines and best practices.
