Job summary

ProMedica Conrad Jobst Tower is seeking a Clinical Pharmacist to perform ambulatory care clinical pharmacist functions and cover multiple ProMedica Pharmacy Medication Management sites as needed.

You will manage referred patients' medication therapy, communicate with patients and other pharmacists, and contribute to the education of pharmacy residents and medical students. This full-time clinical role supports diverse ambulatory settings in Ohio and Michigan.

Qualifications

• PharmD or BS Pharm with post 2006 experience.
• 1 year of pharmacy experience.
• BLS certification.
• Licensed pharmacist in Ohio and Michigan.

Responsibilities

• Provide ambulatory care clinical pharmacist services across sites.
• Manage referred patients' medication therapy and communicate with patients and other pharmacists.
• Contribute to education of pharmacy residents and students.
