The role emphasizes collaboration, data-driven care, and participation in training programs while using EPIC and hospital workflows to coordinate transitions of care and improve health outcomes.

Qualifikationen

• Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD).
• Active Illinois Pharmacist license (IDFPR) required or eligible.
• Experience with ambulatory care or clinical pharmacy desirable.
