Kingman Regional Medical Center's Disease Management Clinic is seeking an Ambulatory Care Clinical Pharmacist. You will operate at the top of a pharmacist’s scope, managing diabetes with insulin pump therapy, anticoagulation, hypertension, and dyslipidemia under a Collaborative Practice Agreement.

The role emphasizes hepatitis C management within a population health initiative, with a multidisciplinary team approach and opportunities for professional growth in a not-for-profit setting.

Qualifications

• PharmD from ACPE-accredited School of Pharmacy.
• Licensed in the State of Arizona within 90 days of hire.
• PGY1/PGY2 residency or Board Certification preferred.

Responsibilities

• Manage disease states under a Collaborative Practice Agreement with prescriptive and lab ordering authority.
• Focus on hepatitis C as part of population health initiatives and perform anticoagulation management.
• Collaborate with multidisciplinary teams (Primary Care, Endocrinology, Oncology, Cardiology, Hepatology) to optimize therapy.

Position Purpose:

The DMC encompasses a team of pharmacists providing disease state medication management services through a Collaborative Practice Agreement with our community practitioners (Primary Care, Endocrinology, Oncology, Cardiology, Hepatology, and more). This is a setting where a multidisciplinary health care team approach is valued and expected. Working at the top of a pharmacist\’s scope of practice, the specialist will manage a variety of disease states through delegated prescriptive and lab ordering authority. These include an emphasis on hepatitis C as part of our population health-based HCV Elimination Initiative, along with anticoagulation, diabetes with insulin pump management, hypertension, and dyslipidemia. Qualifications:

• Education: Graduate with a Doctor of Pharmacy Degree from an ACPE-accredited School of Pharmacy
• License:Licensed in the State of Arizona; required within 90 days of hire date
• Preferred, but not required: PGY1/PGY2 or Board Certification

Work Requirements:

Must be able to distinguish color, sit approximately 37% of the time; stand approximately 30% of the time; walk approximately 33% of the time; balancing, crouching, fingering, grasping, lifting, carrying, pushing, reaching, stooping, and repetitive movement. Able to carry or lift weight in the range of 25-75 pounds.
