Job summary

Atlantic Health System is seeking a Per Diem Clinical Pharmacist focused on ambulatory cardiology at the Overlook Medical Center in Summit, NJ. Qualifications

• Pharmacist license in good standing and PharmD degree.
• Experience in cardiology or ambulatory care preferred.
• Ability to perform medication reconciliation and optimization.

Responsibilities

• Conduct comprehensive medication reviews for cardiology patients.
• Coordinate with interdisciplinary teams for optimized therapy.
• Provide patient education on regimens and adherence.
• Document interventions and patient encounters in the EHR.
