Astrya Global Medical Staffing is hiring a Licensed Clinical Social Worker (LCSW) for a temporary assignment in Grand Junction, Colorado. The position runs 8:00 AM – 5:00 PM, five days per week, on a 6-week term with potential extension. Salary is $46–$49 per hour.

Work as part of an interdisciplinary care team, providing social work services to patients and families, conducting home visits, and advocating for patients while coordinating with nursing, medical staff, and CNAs.

Qualifications

• Minimum of two years healthcare social work experience.

Responsibilities

• Provide social work services to patients and families as part of an interdisciplinary care team.
• Collaborate with social workers, chaplains, nurses, physicians, and CNAs to improve patient care.
• Serve as a patient advocate by providing education and resources to patients and families.
• Conduct home visits and see patients in their personal living environments.
• Work within the community to provide quality care through education, resources, and individualized support.
• Honor the goals and wishes of each patient in care delivery.
• Partner with professional colleagues in nursing homes and assisted living residences when applicable.
• Utilize excellent interpersonal and communication skills when interacting with patients, families, and team members.
• Serve as an ambassador for the organization within the communities served.
• Support patients experiencing aging, illness, and grief with compassion and professionalism.

Astrya Global, a San Diego based medical staffing agency, is hiring a LCSW for a temporary assignment in Grand Junction, Colorado. These positions are 6-week contracts with the ability to extend.

Responsibilities for a LCSW:

• Provide social work services to patients and families as part of an interdisciplinary care team
• Collaborate with social workers, chaplains, nurses, physicians, and certified nursing assistants to improve patient care
• Serve as a patient advocate by providing education and resources to patients and their families
