• Respond to requests for medical social work services, including case management, discharge planning, nursing home/assisted living placements, risk management, crisis intervention, and care coordination.

Responsibilities

• Provide medical social work services to patients in the Acute Care Unit and Primary Care Clinic.

• Social Worker coordinates nursing home and assisted living placements for patients.
