Active Day in Naperville, IL is seeking a Personal Support Worker to provide one-on-one home-based support for children and adults with developmental disabilities. You will implement and document goals and behavioral plans, assist with ADLs, and promote independence while ensuring safety and dignity for every client.
The role requires DSP certification or completion upon hire, CPR/First Aid, valid vehicle, and ability to work with diverse ages.
Overview : The Personal Support Worker will provide one-on-one services to children and adults with developmental disabilities in the Home-Based Support Services program through DHS. Essential characteristics of self-motivation, flexibility and a concierge outlook to provide the highest quality services for our clients.
Description/job Duties : Provide direct support to both children and adults with intellectual disabilities in the home-based program. Collaborate with client support team members and ensure that individuals exercise their rights and are treated with dignity and respect. Responsible for the implementation and documentation of individual goals, objectives , behavior tracking and interventions when applicable in the Implementation Strategy Plan and Behavior Support Plan of each individual served. Assist individuals with their activities of daily living (ADL), promoting independence including, but not limited to, household chores, recreational activities, physical fitness activities, grooming and dressing, money management and community integration. Know the whereabouts of individuals you are directly responsible for at all times. No client can be left unattended for any period of time during your shift. Attend training as required to continue skill development as necessary. Meet the needs of the individual served and their family/guardian(s). Report medical or behavioral concerns and accidents / incidents immediately to parent/guardian and your supervisor. Report any and all allegations of individual abuse or neglect that is reported to or witnessed immediately your supervisor and Office of Inspector General (OIG) . Responsible for collecting data and filling out appropriate documentation each shift . Completion of state required DSP Certification within 90 days of hire . Other duties assigned . Non Direct time will be spent in Naperville office. This can include but not limited to documentation, session prep, trainings , meeting with supervisor etc.
Hours : Tuesday, Wednesday, Thursday Friday 9:00am-2:00pm. MUST BE ABLE TO WOR K FULL SCHE DULED LISTED.
Location : Office is located in Naperville, IL. Client Located in all neighboring areas.
Reports to : Clinical Manager
Must be 21 years of age.
Access to a registered vehicle with proof of insurance to provide transportation for individuals served.
Ability to work with both children and adults.
Current certified Direct Support Person (DSP) or completion of DSP certification upon hire.
CPR and First Aid Certification or completion of certifications upon hire.
Experience and understanding of individuals with developmental disabilities or mental health diagnoses.
Experience with individuals who exhibit maladaptive behaviors.
Successful clearance of local and/or state background checks.
