Job summary

Aultman Health Foundation is seeking a Hospice Aide to deliver compassionate personal care and homemaker services under RN guidance. The role covers assistance with hygiene, mobility, feeding, and basic daily living tasks across hospice settings including patients' homes and facilities.

You will reinforce caregiver education, document care provided, and participate in the interdisciplinary team’s plan of care. Required STNA and BLS credentials, Ohio driver’s license, and reliable transportation.

Qualifications

  • STNA certification required or preferred.
  • One year of home care or related healthcare experience preferred.
  • Current BLS certification and valid Ohio driver's license required.
  • Must have own transportation and provide proof of liability insurance.

Responsibilities

  • Assist with personal hygiene, elimination, feeding and mobility per patient needs and nursing orders.
  • Reinforce patient caregiver education on basic personal care and turning/repositioning.
  • Deliver care across hospice settings including home, inpatient facility, and other facilities as assigned.
  • Communicate findings to the hospice nurse and document patient response to care.
  • Provide homemaker services and light housekeeping and meal preparation as needed.
  • Complete at least 12 hours of in-service training annually and participate in the care plan development.

Job description

PURPOSE OF POSITION

The primary purpose of the Hospice Aide is to provide personal care and homemaker services based on the registered nurse's initial and ongoing assessments of the patient's personal care needs, ability to perform activities of daily living and supervision of care.

  • Assists with personal hygiene, elimination, feeding and mobility according to the patient's needs and the nursing orders
  • Provides support for and reinforces the team's teaching of patient's caregivers on basic personal care, including turning and repositioning patients, making beds and bathing and showering patients as needed
  • Delivers care in all settings where hospice care is provided, including hospice inpatient facility, private residences, assisted living facilities, nursing facilities or group homes as assigned
  • Communicates with the hospice nurse regarding services provided and significant findings regarding the patient's functional status and change in care needs
  • Documents the care provided and the patient's response to care
  • Provides homemaker services, including light housekeeping and meal preparation, in the patient's home according to the patient's needs and the nursing orders
  • Completes and maintains documentation of at least 12 hours of in-service training during each 12-month period
  • Participates with the interdisciplinary team in the development and implementation of the patient's and family's plan of care
  • Displays a positive and professional image and attitude in all relationships with patients, families, peers and in the community
  • Participates in the Quality Assessment Performance Improvement (QAPI) program, as requested
  • Demonstrates flexibility in meeting the needs of the program and responds positively to changes in workflow
  • Complies with agency infection control and safety policies
  • Completes annual competency requirements
  • Consistently communicates requests, suggestions, concerns or problems to the appropriate personnel
  • Consistently completes assigned responsibilities in required time frame and accepts additional duties and responsibilities as requested by Unit Director or designee
  • Protects patient health information according to the HIPPA Privacy Rule and related Aultman policies
  • Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation
  • Supports strategic organizational goals
  • Abides by the Aultman Code of Conduct at all times

Qualifications
  • STNA certification
  • One (1) year experience in home care or other related healthcare experience preferred
  • Current BLS certification
  • Valid Ohio Driver's License
  • Must have own transportation and provide proof of current liability insurance
Working Conditions
  • Hours of operation with shifts as assigned including occasional overtime, on-call or off-shifts (evenings and/or weekends) scheduled as necessary
  • Lunch and break periods must be coordinated with other staff members to maintain adequate staffing during hospice hours of operation
  • Subject to frequent interruptions and changes in priority of duties throughout the day
  • Subject to emergency and other crisis situations
  • May be required to travel in inclement weather
  • Sitting/standing/moving about during working hours (see attached Physical Requirements Addendum for details)
