Job summary

Gentiva seeks a dedicated Certified Home Health Aide (CHHA) for part-time dayshift to assist terminally ill patients in their homes, hospices, or long-term care settings. The role emphasizes compassionate, hands-on care and collaboration with a patient-centered care team.

You will deliver personal care, help with hygiene and mobility, assist in meals, and maintain documentation while following safety protocols and providing emotional support to patients and families.

Qualifications

  • Active CNA license in the state of employment (Required).
  • Current CPR certification (Required).
  • Driver’s license and reliable vehicle with insurance (Required).
  • Six months of direct patient care experience (Required).
  • Comfortable working in home settings, hospice facilities, or long-term care environments.
  • Demonstrated compassion, patience, and ability to handle emotional situations.
  • Willingness to work independently and adapt to changing patient needs.

Responsibilities

  • Provide direct personal care to terminally ill patients.
  • Assist with personal hygiene, grooming, and toileting.
  • Support safe mobility, including use of walkers and wheelchairs.
  • Help with meal preparation, feeding, and light housekeeping.
  • Observe and report changes in the patient’s physical or mental condition.
  • Offer emotional and psychological support to patients and families.
  • Maintain accurate and timely documentation of visits.
  • Attend team meetings and participate in Quality Improvement (QI) activities.
  • Practice infection control and follow safety protocols at all times.

We’re currently seeking a dependable, empathetic, and attentive Hospice Aide to provide essential personal care and emotional support to patients wherever they call home—whether that's a private residence, assisted living facility, or inpatient unit. Compensation may vary within the salary range provided based on several factors, including but not limited to a candidate's geographic location, experience, education, skills, licensure, certifications, internal equity, and department equity. Gentiva provides associates with a comprehensive Total Rewards package, of which base pay is just one component.

Salary Range: $20.00 - $22.00 per hour

Essential Functions

About You

Qualifications – What You’ll Bring:

  • Active CNA license in the state of employment (Required)

  • Current CPR certification (Required)

  • Six (6) months of direct patient care experience (Required)

  • Comfortable working in home settings, hospice facilities, or long-term care environments

  • Demonstrated compassion, patience, and ability to handle emotional situations

  • Willingness to work independently and adapt to changing patient needs

Preferred Background (Not Required):

  • Experience in home health, hospice, palliative care, or oncology

  • Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes

  • Exposure to end-of-life care, wound care, or admissions support
