Adaptive Home Health is seeking a dedicated Registered Nurse (RN) for field-based case management in Detroit. You will conduct comprehensive in-home assessments, manage high-acuity patient care, and document with OASIS, SOC/recert/ROC, and discharge processes.

Qualifications

• Active Michigan RN license.
• Home health / OASIS experience strongly preferred.
• Reliable transportation, valid driver's license, auto insurance.
• Supervising LPNs and HHAs on cases is a plus.

Responsibilities

• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Direct, high-acuity patient care, with a focus on wound care.
• Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.
• Being the face of Adaptive Home Health to patients, families, and referral sources.
