Job summary

Job description

Role Overview

Provide palliative care to adult oncology patients in an independent, collaborative, and interdisciplinary practice model. Collaborate with primary disease management teams and, as a part of an interprofessional Supportive Care team, provide palliative care to patients with various stages of cancer. Facilitate post-acute transitions from inpatient to outpatient care in accordance with patients’ values and goals.

Key Qualifications
  • Active NYS license or be qualified for state licensure
  • Graduate from an accredited PA program
  • NCCPA certification
  • BLS certification
  • Or Current New York State (NYS) RN and NP licenses/registrations
  • Master’s degree in nursing, prescriptive privileges (or eligibility)
  • Related National NP certification
  • Experience in hospice & palliative care and/or graduate from an accredited hospice & palliative care fellowship strongly preferred

Additional Information

Location: Main Hospital Campus, 1275 York Avenue, New York, NY

Hours: Flex 12‑hour shifts – including weekends and holidays; 13 shifts per 4‑week schedule (Days). Training period will include 10‑12 hour shifts 3‑4 days/week with rotation.

Reporting to: Director, APP

Pay Range: $155,500–$197,400 annually

Benefits Pay Range: $136,400.00–$225,100.00; FSLA Status: Exempt
