Clear Destination Inc. is seeking a Home-Based Primary Care Advanced Practice Provider to serve as the primary clinician for a patient panel in homes and facilities across Kentucky.

The role includes planning, directing, and evaluating care, performing exams, managing chronic and acute conditions, and writing prescriptions within scope. Qualified candidates will be licensed as a Nurse Practitioner or Physician Assistant in Kentucky, with ACNP/AGNP/FNP certification and BLS.

Qualifications

• Graduate of an accredited Master’s or Doctoral program in Nursing or a Physician Assistant Program.
• Current certification as an Advanced Practice Registered Nurse (ACNP, AGNP, FNP) or Physician Assistant.
• Licensed as a Nurse Practitioner or Physician Assistant in Kentucky.
• Maintains current BLS certification.
• Experience in Hospice and Palliative care preferred with strong pain/symptom management.

Responsibilities

• Serve as primary care clinician for a patient panel in homes, assisted living facilities, and personal care facilities.
• Develop, implement, and communicate individualized plans of care; may contribute to protocol development and quality initiatives.
• Collaborate with supervising physician and care team to deliver comprehensive care; perform exams, treat illnesses, order tests, write prescriptions, and perform procedures within scope.

Schedule: Monday-Friday 8:00am to 5:00pm with on-call/afterhours responsibilities

Position Summary:

The Home-Based Primary Care Advanced Practice Provider serves as the primary care clinician for a patient panel residing within homes, assisted living facilities and personal care facilities. The Nurse Practitioner plans, directs, and evaluates care for assigned patients in assisted living facilities or their place of residence. This care would include examining and treating chronic and urgent episodic illnesses, injuries, accidents, and other injuries/ illnesses. Performs selected laboratory testing, referrals to physicians, as needed, writes prescriptions, and performs procedures within the scope of expertise, protocols and available equipment. Works in collaboration with the facility’s supervising physician and other care providers to provide care and treatment services for all assigned patients.

This position plays a key role in developing, implementing, and communicating individualized plans of care and may contribute to clinical protocol development, education, and quality improvement initiatives related to palliative and end-of-life care.

Position Qualifications:

• Graduate of an accredited Master’sor Doctoralprogram in Nursing ora Physician AssistantProgram.

• Demonstrates in-depth, current clinical knowledge withinareaof specialty and the ability to translate professional and organizational goals into effective practice.
• Skilled in supporting patients and families experiencing emotional stress; respectful of diverse lifestyles, values, and beliefs.
• Commitment to the evidence-based practice, research utilization, and continuous professional development.
• Ability to organize and utilize resources efficiently and effectively.
• Strong interpersonal, communication,collaboration,and conflict-resolution skills.
