Prevea Health in Wisconsin is seeking an Advanced Practice Provider (NP or PA) to deliver care under physician supervision. This role is 32 hours per week and focuses on health promotion and disease prevention.

You will assess, educate, coordinate care, manage cases, and document in the EMR. Requires master's level NP or PA program and Wisconsin licensure, plus CPR/BLS.

Qualifications

  • Graduate of a master's level nurse practitioner program OR graduate from an accredited physician assistant program. Required
  • Licensure as NP or PA in Wisconsin with necessary certifications. Upon hire required
  • CPR/BLS certification from an approved program. Required

Responsibilities

  • Provides assessment, evaluation, and management to patients with minor, acute, chronic, & episodic illness & injuries.
  • Collaborates with patients to review their health background and determine needs, advising on future health plans.
  • Provides case management services to arrange or refer patients to needed services.
  • Conducts telephone follow-ups and provides health counseling and instructions.
  • Orders diagnostic tests, prescribes medications, and other non-pharmacological treatments.
  • Maintains and reviews patient records in the electronic medical record.
  • Collaborates with physicians to develop and implement patient care plans.

Job Summary

The Advanced Practice Provider at Prevea is an advanced practice nurse (NP) or physician assistant (PA) prepared to deliver health care services to patients, families and groups emphasizing the promotion of health and prevention of disease under direct supervision and responsibility of physicians. The scope of responsibilities includes collaborating and coordinating with physicians and other health professionals regarding clinical practice, education, consultation, research and program development.

What you will do
  • Provides assessment, evaluation, and management to patients with minor, acute, chronic, & episodic illness & injuries. Including performance of follow up exams if required.
  • Collaborates with patients to review their health background and determine physiological, emotional, and environmental needs, using this information to advise patients and their families on future health plans and illness prevention.
  • Provides case management services to assist clients and families with the arrangement for, or refer patients to needed services.
  • Conducts telephone calls back to patients requiring follow-up care or provide test results. Also provides health counseling and education to patients and their families about their healthcare and any follow-up instructions.
  • Provides services, which may include the ordering of appropriate diagnostic and laboratory tests, the prescription of pharmacological agents, and other non-pharmacological treatment modalities.
  • Maintains and reviews patient records, charts, and other pertinent information, documenting all details in the electronic medical record.
  • Collaborate with physicians and other providers to develop and implement patient care plans to ensure the best possible care and outcomes of patients.
Education Qualifications
  • Graduate of a master's level nurse practitioner program OR graduate from an accredited physician assistant program. Required
Experience Qualifications
  • 1-3 years work experience in a clinical setting within a health care organization Preferred
Skills and Abilities
  • Knowledge of the information and techniques required to diagnose and treat human injuries and diseases, including understanding symptoms, treatment options, drug properties and interactions, and preventive healthcare measures.
  • Demonstrate organizational skills that allow timely patient appointments and appropriate required follow up.
  • Presents a courteous and service oriented image through positive and helpful interaction with patients and families, physicians and co-workers.

Licenses and Certifications
  • Individual must posses either the licenses/certification details of a Nurse Practitioner or a Physician Assistant.
  • Nurse Practitioner: Current Wisconsin RN license or compact equivalent (nurses must maintain or update their primary state of residence to Wisconsin to ensure continued eligibility under the compact) with certification through American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioner Certification Board (AANPCB). Upon Hire Required
  • Physician Assistant: Licensed by the state of Wisconsin as a Physician Assistant. Certification from the National Commission on Certification of Physician Assistant. Upon Hire Required
  • CPR/Basic Life support for Healthcare Providers certification from approved program. Required
