What will you do?
This role is part of the OSU Geriatrics team and provides care to patients in skilled nursing facilities, including post-acute skilled patients and residents with dementia and long-term care needs. The APP functions as a primary care provider during the resident's stay, with responsibilities including medication management, diagnostic imaging orders, laboratory review, discharge planning, and ongoing clinical management. The position also participates in a rotating call schedule with the team.
