Nicklaus Children's Health System is seeking an APRN and Nurse Coordinator for the Craniofacial (CF) Program. The role collaborates with supervising physicians to provide advanced care for CF program children in two settings: the craniofacial multidisciplinary program within the Brain Institute and inpatient referrals.
Responsibilities include coordinating craniofacial team assessments, developing multidisciplinary treatment plans, and overseeing patient navigation, outreach, and registry data
The APRN and Nurse Coordinator CF Program, in association with the supervising physician, assumes responsibility for providing comprehensive care at an advanced practice level to CF Program children with craniofacial differences. The APRN and Nurse Coord CF Program will provide comprehensive assessment and management for such children in two distinct, but coordinated, settings: the craniofacial multi-disciplinary program at Nicklaus Children’s Hospital, housed within the Brain Institute, and inpatient referrals. APRN multi-disciplinary team functions are to include, but are not limited to, the coordination of craniofacial team clinical assessments and patient care, including the composition of multidisciplinary summary treatment plans, facilitation of diagnostic procedures such as nasopharyngoscopy evaluations with speech and language pathologists, and performance of inpatient consultations. Nurse and CF Program Coordination functions include navigation, referral source building and communication, community outreach, Referring PCP and patient/family Team Summary communication, registry data entry, oversight of quality improvement activities, and healthcare transition.
