Job summary

Eskenazi Medical Group is seeking a licensed APRN to plan, coordinate, and evaluate patient care across EMG venues. The role includes diagnostic and therapeutic interventions, prescribing within approved authority, and collaborating with physicians.

You will conduct comprehensive assessments, order and interpret tests, develop care plans, and educate patients while ensuring quality improvement and adherence to NCQA standards.

Qualifications

• Must hold a Master of Science in Nursing and an Indiana RN license.
• Collaborative agreement with affiliated physician group required.
• Current Indiana CDS registration and prescriptive authority as applicable.

Responsibilities

• Provide history, physical assessments and ongoing care plans.
• Order, conduct and interpret diagnostic tests as needed.
• Prescribe medications within approved prescriptive authority.
• Educate patients and coordinate referrals to specialists.
• Participate in quality improvement, committees and practice governance.
• Maintain documentation per NCQA and Eskenazi Health standards.
• Identify and address patient and community needs across care continuum.
• Lead as an educational resource for EMG staff and teams.
• Ensure age-appropriate care across the life span.
