SSM Health DePaul Hospital in Bridgeton and SSM Health St. Joseph West Hospital in Lake St. Louis are recruiting an experienced Nurse Practitioner or Physician Assistant for our Complex Care Clinic.

This is a PRN/Fill-In role offering a competitive rate based on experience and malpractice coverage, with no additional benefits. Outpatient duties include pre-surgery evaluations, coordinating care with PCPs or specialists, and early post-discharge follow‑ups to prevent readmissions.

Qualifications

• Master's degree required for NP or PA, with relevant clinical training.
• 2+ years experience as NP/PA in Internal Medicine or Adult Primary Care.
• Missouri license as APRN or PA certification and RN license.

Responsibilities

• Outpatient clinic coverage with a PRN/fill-in arrangement.
• Perform pre-surgery evaluations and determine optimization needs.
• Coordinate care with PCPs or specialists; limited testing and prescriptions.
• Post-discharge transitional care within the first week to prevent readmissions.
• Demonstrate strong evaluation and assessment skills.
• Mentorship and collaborative team environment.

Requirements

• Missouri registered nurse license and advanced practice nurse licensure or certified physician assistant

Job Highlights

• Outpatient only covering a minimum of two shifts per month at SSM Health St. Joseph West Hospital in Lake St. Louis and covering provider PTO and sick days as available at both SSM Health St. Joseph West Hospital in Lake St. Louis and SSM Health DePaul Hospital in Bridgeton.
• Complete pre-surgery evaluations and determine optimization needs based on algorithms and clinical pathways
• We focus on optimizing medical conditions and often coordinate care with patients' PCP or specialiststhere is limited ordering of testing and medications with this portion of the role
• The transitional care is a post-discharge clinic focused on preventing hospital readmissions. We evaluate patients within the first week post-discharge, assess status, and treat/refer as needed
• This role requires strong evaluation and assessment skills
• Limited diagnostic workups and prescriptions are involved in this setting
• Strong department onboarding and mentorship
