Job summary

Longevity Health is seeking an Advanced Practice Provider to deliver patient-centered care for institutionalized beneficiaries in the Raleigh, NC area. The APP will work with contracted providers to ensure timely, coordinated care and optimal outcomes for a complex patient population.

Requirements include an active NP/PA license, national certification (AANP/ANCC/AACN or NCCPA), a valid state driver’s license, and Medicare provider number.

Qualifications

  • Experience collaborating with physician groups to coordinate beneficiary medical management.
  • Strong communication with patients, families, and providers.
  • Able to function independently in a team-based care model.
  • Geriatric, hospice or palliative care exposure is preferred.

Responsibilities

  • Collaborate with primary provider groups to address beneficiary medical management needs.
  • Prevent hospitalizations and support care transitions.
  • Develop and update goals of care and advanced care plans.
  • Complete medication reconciliation and identify patients for palliative/hospice pathways.
  • Establish relationships with patients, facilities and provider partners.

About the Role

TheAdvancedPracticeProvider(APP)playsavitalroleinadvancingLongevityHealth’s commitment to improving the health and quality of life of institutionalized beneficiaries. This individual will deliver dignified, patient-centered care that addresses unique medical, social and emotional needs while exemplifying the organization’s missions and values.

The Advanced Practice Provider (APP) will be assigned to a specified geography and will be responsible for delivering ongoing care based on acuity, clinical need and an established risk stratification protocol. In partnership with contracted provider groups, the APP serves as an additional clinical resource to ensure beneficiaries receive timely, coordinated care that promotes best possible outcomes.

  • StateunrestrictedlicensedAdvancedPracticeNurse,NationalBoardcertification(AANP,ANCCor AACN)asanAdult,Family,GeriatricorPrimary Carenursepractitionerrequired.Willallowacute care nurse practitioner with nursing home/geriatric or hospice palliative experience OR state unrestricted licensed Physician’s Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience
  • Musthaveanactivenationalprovidernumber(NPI)andanactiveMedicareprovider number
  • MustbeeligibletoparticipateintheMedicareprogramasaproviderand registered
  • Valid state driver’s license with a good driving record and proof of automobile insurance required
  • A utoinsurancecoverageasrequiredbyyourstateofresidence.Newemployeeswillbe responsible for providing proof of coverage prior to hire and on an ongoing basis

Education and Training:

  • Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program

Knowledge and Experience

  • Minimum of 6 months experience as a Nurse Practitioner/Physician’s Assistant AND has some experience with geriatric/hospiceor palliative care population priorto becomingan NP/PAORis a licensed and board-certified new graduate NP/PA supervised by an experienced NP/PA in the Longevity Health New Graduate Fellowship programfor6 months andsuccessfully completes all competencies for independent practice as a Longevity Health CareOne ACO APP
  • The ability to engage and communicate effectively with physicians, patients, family members—excellent people skills
  • Self-directedbutfunctionswellinateam environment
  • Excellent clinical skills—assessment, diagnosis, treatment, prescribing specifically for the chronically ill population
  • C anfunctionindependently,demonstratedcriticalthinkingskills,organization,andprioritization skills
