Job summary

Heart Clinic of Hammond in Hammond, LA is seeking a compassionate Nurse Practitioner to provide high-quality cardiovascular care in outpatient and hospital-based settings.

The NP will work with Dr. Mikdadi to deliver comprehensive care, including clinic visits and rounds at North Oaks Hospital and PAM, with weekend call rotation and major/minor holidays.

Qualifications

• Active NP license in Louisiana or eligibility in state.
• Cardiology experience preferred for outpatient and hospital settings.
• Ability to work independently within a physician-led team.

Responsibilities

• Conduct outpatient clinic visits including routine follow-ups and chronic disease management.
• Evaluate ongoing cardiac concerns and coordinate care with physicians.
• Provide patient education and assist with diagnostic testing.
• Support continuity of care post-hospitalization and transitions to outpatient management.
• Perform rounds at hospital facilities and assist with hospital-based care.
• Collaborate to ensure efficient patient flow and a well-coordinated clinic experience.
• Participate in weekend call rotation and holiday coverage.

Position Overview

We are seeking a compassionate and motivated Nurse Practitioner to join Heart Clinic of Hammond in Hammond, Louisiana. The NP will work closely with Dr. Mikdadi to provide high-quality cardiovascular care, including outpatient clinic visits and hospital rounding at North Oaks Hospital and Post Acute Medical (PAM). This is an excellent opportunity to join a well-established cardiology practice with a strong emphasis on patient-centered, comprehensive care.

Key Responsibilities

• Conduct outpatient clinic visits, including routine follow-ups, chronic disease management, and evaluation of ongoing cardiac concerns.
• Support continuity of care by following patients post-hospitalization and integrating them into ongoing outpatient management.
• Provide clinical support within the cardiology clinic, including patient education and assistance with diagnostic testing.
• Collaborate with physician to ensure efficient patient flow and a well-coordinated clinic experience.
• Perform rounds at North Oaks Hospital and Post Acute Medical (PAM).
• Support smooth transitions from hospital to outpatient care.
• Participate in weekend call rotation, along with one major and one minor holiday annually.

Qualifications

• Active Nurse Practitioner license in the state of Louisiana.
• Prior cardiology experience preferred.
• Strong clinical skills in both outpatient and hospital-based settings.
• Ability to work independently while contributing to a collaborative, physician-led team.
