Job summary

Wood River Health in Hope Valley, RI is seeking a full-time Advanced Practice Provider to join our primary care team. The APP will provide full-range primary, urgent, and acute care within their scope, collaborating with physicians and other health staff.

Required: graduate/post-graduate training, 2+ years in primary care, RI license/CSR/DEA, CPR, and ongoing CME. NHSC loan repayment site with generous benefits and scheduling options.

Qualifications

  • Completion of a graduate or post-graduate program.
  • A minimum of 2 years of primary care experience in either a family medicine or primary care outpatient practice is preferred.
  • Ongoing CME to update skills/therapies, as per Rhode Island Department of Health regulations.
  • Ability to effectively use MS Office Suite, Internet, and email.
  • Cultural sensitivity necessary to work with a diverse patient and staff population.

Responsibilities

  • Provide full range of primary care, urgent care, and acute care within scope and credentialing.
  • Prescribe or administer treatment, therapy, medication, vaccination, and other care.
  • Order, perform, and interpret tests and analyze records for diagnosis.
  • Coordinate care with nurses, staff physicians, and other providers.
  • Manage patients with substance use disorders including MAT and Suboxone.

Salary Range: $120,000.00 To $134,000.00 Annually

Our APP's are an integral part of our provider team, providing the full range of primary care, urgent care, and acute care according to their training scope and credentialing. This includes responsibility for examination, diagnosis, and treatment of patients with acute, episodic, and chronic care, independently and as part of a healthcare team. The APP collaborates with the Chief Medical Officer and Associate Medical Director in the management of care and participates in administrative and clinical meetings as scheduled.

ESSENTIAL DUTIES; include but are not limited to:

  • Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.
  • Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patients’ condition.
  • Collect, record, and maintain patient information, such as medical history, reports, and examination results.
  • Monitor patients’ conditions and progress and reevaluate treatments as necessary.
  • Explain procedures and discuss test results or prescribed treatments with patients.
  • Advise patients and community members concerning diet, activity, hygiene, and disease prevention.
  • Refer patients to medical specialists or other practitioners when necessary.
  • Coordinate work with fellow Advanced Practice Providers, Staff physicians, Chief Medical Officer, Registered Nurses, Social Workers, Rehabilitation Therapists, Pharmacists, and other health care providers.
  • Assess and treat patients with substance use disorders, initiating and managing medication-assisted treatment (MAT), and prescribing and monitoring medication, including Suboxone. Works collaboratively within an integrated care team to support whole-person, high- quality care.
  • Direct and coordinate activities of nurses, students, assistants, specialists, therapists, and other medical staff.
  • Counsel patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter medications, and herbal remedies
  • Recommend interventions to modify behavior associated with health risks.
  • Detect and respond to adverse drug reactions, with special attention to vulnerable populations such as infants, children, pregnant and lactating women.
  • Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients’ individual circumstances.
  • Plan, implement, or administer health programs or standards in hospitals, businesses, or communities for prevention or treatment of injury or illness.
  • Peer review and quality improvement activities.
  • Collaborates cooperatively with all personnel of the agency.
  • Maintain strict confidentiality in all matters
  • Operate within the scope of the Health Information Portability and Accountability Act to safeguard the privacy of protected patient health information
  • Enliven and support the mission, vision, and values of Wood River Health
  • Adhere to organizational policies and procedures and Wood River Health Compliance Program Standards
  • Performing other duties as assigned to meet business needs

Our schedules include a late night (until 8pm) per week, rotating Saturday shift (9am-2pm), and rotating call schedule. Providers have a set schedule which includes shifts in both our Hope Valley and Westerly locations.
Full time and part time schedules available. 4 day workweek available.
