Qualifications

  • Licensed or license-eligible Nurse Practitioner in Rhode Island.

  • Strong ability to communicate across teams and synthesize multidisciplinary information.

Job description

Summary

The Inpatient PM&R Consult Service supports acute care patients at Rhode Island Hospital and The Miriam Hospital whose recovery depends on thoughtful, early, and highly coordinated rehabilitation assessment. This service works at the intersection of complex medical care, neurologic recovery, trauma, orthopedic and postsurgical management, discharge planning, and systems-based care.

The Advanced Practice Provider will work alongside and in support of a physiatrist with subspecialty training in neurorehabilitation, helping to deliver high-level consultative rehabilitation care in a fast-paced tertiary care academic environment. This role is designed for a clinically strong, operationally savvy, and highly collaborative PA or NP who is energized by medically complex patients, interdisciplinary care, and the opportunity to help shape an expanding hospital-based rehabilitation service.

  • Provide high-quality, evidence-based assessment and co-management of acute hospital patients with complex medical, neurologic, trauma, orthopedic, and postsurgical conditions requiring rehabilitation expertise.
  • Perform comprehensive inpatient rehabilitation evaluations, including functional assessment, mobility and self-care needs, rehabilitation candidacy, equipment needs, and discharge planning recommendations.
  • Collaborate closely with referring medical, surgical, and subspecialty teams to identify rehabilitation needs early, reduce avoidable hospital length of stay, and facilitate safe, efficient transitions of care.
  • Help determine the most appropriate next level of care, including discharge to home with outpatient PM&R follow-up, other post-acute rehabilitation services, or admission to Vanderbilt Inpatient Acute Rehabilitation at Newport Hospital within the Brown University Health system.
  • Serve as a key liaison among the acute hospital teams, case management, therapy services, social work, nursing, and the inpatient consultative physiatrist to ensure timely communication and coordinated execution of care plans.
  • Assist with administrative and care-coordination needs related to inpatient rehabilitation planning, documentation, transition management, and communication across sites and services.
  • Provide backup clinical support on the acute rehabilitation unit, as needed, to support staffing and continuity of rehabilitation care.

Responsibilities
  • Evaluate and follow medically complex inpatients referred for PM&R consultation.
  • Develop and document rehabilitation-focused assessments and plans in partnership with the supervising physiatrist.
  • Identify functional impairments, activity limitations, rehabilitation goals, barriers to discharge, and post-acute care needs.
  • Participate in interdisciplinary discussions with hospital medicine, neurology, neurosurgery, trauma, orthopedics, intensive care, case management, nursing, PT, OT, SLP, and social work.
  • Support timely implementation of recommendations related to mobility, therapy intensity, precautions, equipment, spasticity-related issues, neuro-recovery needs, and longitudinal rehabilitation planning.
  • Monitor patient progress and update recommendations as clinical status evolves.

Performs other duties as assigned.

Education

Licensed or license-eligible Nurse Practitioner in Rhode Island.

Experience
  • Prior experience caring for complex adult patients in an acute care hospital, consultative service, inpatient rehabilitation setting, or other high-acuity academic medical center environment.
  • Demonstrated strength in assessing medically complex and functionally impaired patients, synthesizing multidisciplinary information, and communicating clearly across teams.
  • Strong interest in neurorehabilitation, medically complex rehabilitation, hospital-based PM&R, and transitions-of-care work.
  • Ability to work with a high degree of professionalism, efficiency, clinical judgment, and accountability in a fast-paced tertiary care setting.
  • Commitment to patient-centered care, interdisciplinary teamwork, operational excellence, and continuous quality improvement.

$127,504.00-$161,782.40
