Job summary

HealthspanMD Preventive Cardiology in Phoenix, AZ seeks an experienced Physician Assistant or Nurse Practitioner to deliver longitudinal outpatient care within Healthspan360 programs, under physician leadership.

You'll synthesize data, implement personalized plans, coordinate a multidisciplinary team, and help evolve care delivery beyond volume clinic norms, while maintaining HIPAA-compliant documentation and a high-touch patient experience.

Qualifications

  • Current PA-C certification or NP certification appropriate to adult outpatient care.
  • Active, unrestricted Arizona license and prescribing authority, or credible ability to obtain them quickly before practice.
  • Graduate degree from an accredited PA or NP program.
  • At least 3 years of post-training clinical experience in outpatient internal medicine, family medicine, preventive cardiology, endocrinology/metabolic medicine, geriatrics, obesity medicine, or a related field.
  • Strong clinical reasoning, medication-management, documentation, care-coordination, and patient-education skills.
  • Comfort with high-touch service while maintaining evidence, ethics, boundaries, and efficient follow-through.
  • BLS certification; ability to obtain ACLS if requested. DEA registration required if the scope includes controlled substances.

Responsibilities

  • Provide comprehensive outpatient care including history, examination, assessment, diagnosis, orders, treatment, prescribing, monitoring, and documentation.
  • Help members implement and update Healthspan360 plans spanning cardiometabolic health, prevention, brain health, physical performance, energy, sleep, body composition, medications, and behavior change.
  • Review and synthesize advanced laboratory testing, imaging, cardiovascular assessments, DEXA, VO2, cognitive data, and functional-performance results.
  • Manage medications and physician-approved treatment pathways, with disciplined indications, safety monitoring, consent, and escalation.
  • Triage and respond to results, refills, and member questions; support the practice's one-business-day communication standard and same/next-day urgent access model.
  • Coordinate closely with Drs. Lisa and Robert Hurst, the preventive-cardiology APP, physical therapy/performance, Healthspan Strategist, member-experience team, and outside specialists.
  • Maintain clear action plans, close clinical loops, and ensure every assigned member knows the priority, owner, and next step.
  • Build and improve practical clinical workflows, templates, education, quality checks, and program measures.
  • Recognize emergencies and complexity early and escalation appropriately; maintain HIPAA-compliant documentation and communication.

The opportunity

We are seeking an experienced Physician Assistant or Nurse Practitioner to provide outstanding longitudinal care and help guide the Healthspan360 membership program under physician medical leadership. This role combines excellent outpatient APP practice with program ownership: synthesizing complex data, implementing personalized plans, coordinating a multidisciplinary team, and making sure recommendations turn into measurable action.

This is a fit for a clinically grounded APP who wants more time with patients, enjoys prevention and complex data, communicates exceptionally well, and is energized by improving how care is delivered. It is not a volume-clinic role and it is not an unsupervised 'wellness sales' role, you will be working under the supervision of a physician.

What you will do

Provide comprehensive and follow-up outpatient care, including history, examination, assessment, diagnosis, orders, treatment, prescribing, monitoring, and documentation within Arizona scope and HealthspanMD protocols.

Help members implement and update individualized Healthspan360 plans spanning cardiometabolic health, prevention, brain health, physical performance, energy, sleep, body composition, medications, and behavior change.

Review and synthesize advanced laboratory testing, imaging, cardiovascular assessments, DEXA, VO2, cognitive data, and functional-performance results.

Manage appropriate medications and physician-approved treatment pathways, with disciplined indications, safety monitoring, consent, and escalation.

Required qualifications

Current PA-C certification through NCCPA or current NP national certification appropriate to adult outpatient care (such as FNP or AGPCNP).

Active, unrestricted Arizona license and prescribing authority, or a credible ability to obtain them on an accelerated timeline before clinical practice.

Graduate degree from an accredited PA or NP program.

At least 3 years of post-training clinical experience in outpatient internal medicine, family medicine, preventive cardiology, endocrinology/metabolic medicine, geriatrics, obesity medicine, or a closely related field.

Strong clinical reasoning, medication-management, documentation, care-coordination, and patient-education skills.

Comfort with high-touch service while maintaining evidence, ethics, boundaries, and efficient follow-through.

BLS certification; ability to obtain ACLS if requested. DEA registration required if the assigned scope includes controlled substances.

Preferred qualifications

5+ years of relevant outpatient experience and a track record of safely managing medically complex adults.

Experience with advanced lipid and cardiometabolic risk, weight management, hypertension, diabetes/insulin resistance, hormone care, cognition, sleep, or performance medicine.

Experience in concierge, membership, executive-health, preventive, or longitudinal-care models.

Evidence of building a pathway, service line, dashboard, protocol, or major workflow—not merely participating in one.

Arizona PA collaborative-practice certification based on 8,000 qualifying hours, when applicable, or extensive NP autonomous-practice experience.

Physicians can trust that routine care and follow-up are accurate, complete, documented, and escalated when needed.
