Job summary

Hopscotch Primary Care in Marion, North Carolina is seeking an experienced primary care clinician to deliver comprehensive care to a defined panel, including in-person and telehealth visits, aligned with a value-based care model.

You will work with an on-site interdisciplinary team and utilize HCC documentation, ICD-10 coding, and HRAs. A valid US medical license and work authorization are required for employment.

Qualifications

• Three years of experience in a primary care specialty (internal medicine, geriatrics, or family medicine).
• Active, non-probationary state medical license and US work authorization.
• Experience with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• Experience with multiple software platforms and data handling in a clinical setting.

Responsibilities

• Deliver comprehensive primary care services to a defined patient panel, including office and telehealth visits.
• Collaborate with physicians, community health workers, case managers, pharmacists, and others to achieve high-quality outcomes.
• Complete annual wellness visits and risk assessments.
• Follow evidence-based guidelines and innovate care models for patient needs.
• Meet high benchmarks for quality metrics, patient experience, and utilization.

What You'll Do

• Deliver comprehensive primary care services to a set panel of patients, including office visits and telehealth visits for routine and acute primary care issues
• Work closely with physicians, community health workers, case managers, pharmacists, and others to deliver high-quality outcomes
• Completion of comprehensive annual wellness visits and risk assessments
• Practice with a "continuous-improvement" mindset, following evidence-based guidelines and innovating on care models that solve real problems for our patients
• Meet high benchmark for results on quality metrics and outcomes, including patient experience, STARS/HEDIS, and utilization

About You

• A minimum of three years' experience in a primary care specialty (internal medicine, geriatrics, family medicine)
• Experience with HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding and HRA (Health Risk Assessments)
• Experience working with multiple software platforms, electronic health records, spreadsheet development and navigation, data processing, organizational skills, and ability to manage multiple priorities appropriately
• An active, non-probationary state medical license
• US work authorization
