About the Role

In this role, you'll be responsible for a panel of enrolled members receiving nursing home care. You'll be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members. As part of your role, you'll conduct comprehensive assessments and develop individualized care plans for each member. You'll actively participate in nursing facility care team meetings, maintain frequent communication with members' families and loved ones, and ensure the care team stays aligned with members' goals and needs.
