Job summary

SMG System Medical Group in Charleston, WV seeks an Advanced Practice Professional with surgical urology expertise to provide patient-centered care across inpatient, peri-operative and ambulatory settings. The APP collaborates with surgeons, nurses and the health care team to assess, diagnose, and manage patient plans of care.

The role includes rounds, pre/post-operative evaluations, discharge planning and follow-up coordination in a dynamic urology service.

Qualifications

  • Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing.
  • National Certification by one of the recognized APRN Certification Centers.
  • Licensure; or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
  • APRN will need to be privileged as First Assists and a RNFA certification.
  • BLS Certification required with additional Advanced Life Support training at the discretion of the department.
  • Surgical and/or urology experience preferred.
  • Eligible new graduates will be considered.
  • Acute Care or FNPs considered.

Responsibilities

  • Manages comprehensive and individualized care for surgical urology patients across inpatient, peri-operative and ambulatory settings.
  • Completes daily rounds; consults and/or pre-procedure/urology evaluations.
  • Develops a comprehensive plan of care with the clinical team and coordinates peri-operative care and follow-up.
  • Performs necessary procedures following established guidelines and protocols.
  • Collaborates with physicians, nurses and other staff to ensure coordinated patient care.

The Advanced Practice Professional (APP) provides high-quality,patient-centered care as a member of the interdisciplinaryhealthcare team. Working collaboratively with physicians, nurses,and other healthcare professionals, the APP assesses, diagnoses,treats, and manages patients within their scope of practice andarea of specialty. The APP is responsible for deliveringevidence-based care, promoting positive patient outcomes,maintaining professional and regulatory standards, and supportingthe clinical, educational, and operational goals of the departmentand organization.

POSITION SUMMARY

The Advance Practice Professional (APP)is an individual with clinical/surgical urology expertise. The APPcompletes comprehensive patient health evaluations (includingbiological, social and psychological assessments) in collaborationwith the appropriate clinical/faculty team. The APP assesses, andcoordinates the educational and psychosocial needs of the surgicalurology patient and caretakers. The APP develops and reviews theplan of care with the other members of the clinical team,participates in directing that care, and coordinates peri-operative care discharge planning (as applicable) and appropriatepost hospital and ambulatory follow up. The APP participates inthe education of residents and medical/nursing students andperforms diagnostic procedures and therapeutic procedures. The APPparticipates in all aspects of the global management of thesurgical urology patient throughout the system.

Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable. Suchservice may include, but are not limited to .

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER
  • Graduate of a CCNE accredited nursing program with Master’s Degreein Nursing.
  • National Certification by one of the recognized APRN CertificationCenters.
  • Licensure; or eligibility for licensure, as a professionalRegistered Nurse and Advanced Practice Registered Nurse in WestVirginia.
  • Department Specific : APRNs will need to be privileged as FirstAssists and a re required to have RNFA certification .
  • BLS Certification required with additional Advanced Life Supporttraining at the discretion of the department.
EXPERIENCE
  • Surgical and/or urology experience preferred.
  • Eligible new graduates will be considered .
PREFERRED QUALIFICATIONS

MINIMUM QUALIFICATIONS FOR PHYSICIAN ASSISTANT
  • Bachelor ’s Degree or Master’s Degree as Physician Assistantrequired.
  • Graduate of an ARC-PA approved Physician Assistant (PA) trainingprogram required.
  • Certification by the National Commission for Certification ofPhysician Assistants required.
  • Must be licensed or eligible for licensure by the WV Board ofMedicine or WV Board of Osteopathic Medicine.
  • BLS Certification required with additional Advanced Life Supporttraining at the discretion of the department.
EXPERIENCE
  • Surgical and/or urology experience preferred.
  • Eligible new graduates will be considered .
PREFERRED QUALIFICATIONS
  • Master’s level degree as Physician Assistant preferred.
CORE DUTIES AND RESPONSIBILITIES
  • Manages comprehensive and individualized care for surgical urologypatients and their families in the inpatient phase ofhospitalization (as applicable), immediate post discharge, and inthe ambulatory setting.
  • Completes daily rounds ; consults and/or pre-procedure/surgicalurology evaluations.
  • Completes integrated assessments.
  • Participates in peri -operative management of patient which mayinclude, but not limited to, 1 st assisting in the operativesuite.
  • With the collaboration of surgical urology faculty or by followingestablished clinical Guidelines and Protocols, orders additionaltests or consults as deemed appropriate.
  • Performs and communicates results of diagnostic tests followingestablished clinical guidelines and scope of practice.
  • Consults and communicates with clinic and hospital staff regardingabnormal findings in the evaluations of patients.
  • Performs succinct comprehensive clinical evaluations and documentsfindings in medical record.
  • Facilitates patient discharge/follow up by coordinating appropriateservices and tests prior to discharge necessary for outpatientfollow-up appointments in the clinic (as applicable).
  • Performs necessary procedures following established clinicalguidelines and protocols for urological surgery and scope ofpractice (SOP).
  • Manages comprehensive and individualized care through the inpatientand immediate post-discharge phase of the patient’s hospitalizationand in the outpatient setting (as applicable).
  • Develops a comprehensive individualized plan of care for surgicalurology patients, which includes teaching, dischargeplanning/follow-up and necessary referral to other members of thehealth care team as necessary.
  • Demonstrates expertise in caring for patients across the agecontinuum of their educational experience and certification.
  • Participates in physician rounds and consultations (asapplicable).
  • Identifies unstable or complex surgical urology patient caresituations and facilitates appropriate intervention in the clinicand/or hospital setting.
  • Facilitates patient discharge and/or postoperative care bycoordinating appropriate services and tests prior to scheduledoutpatient follow up appointments (as applicable).
  • While in the clinic setting, assists in the collection of outcomemeasures (as applicable).
  • Recognizes emergency situations and initiates effective emergencycare.
  • Evaluates the effectiveness of the surgery patient/family teachingand collaborates with the nursing and clinical staff in thecompletion of education plan.
  • Assists in the collection of outcome measures (asapplicable).
  • Demonstrates critical thinking and diagnostic reasoning skills inclinical decision making by the identification, evaluation andmanagement of the individual needs of patients/families.
  • Demonstrates the ability to utilize equipment in a safemanner.
  • Bases decisions made/actions taken on the relevant scientificprinciples, established standards of care and/or researchfindings.
  • Utilizes teaching/learning theory and appropriate teachingstrategies in the provision of education to nursing staff, hospitalstaff, students, patients, families and the community.
  • Promotes an environment that facilitates learning by responding toappropriate patient/family preferences, priorities and readiness tolearn.
  • Updates own knowledge base throughout the year.
  • Completes all identified clinical competencies.
  • Serves as a resource person for staff caring for the surgicalurology patient.
  • Provides educational programs to professional and paraprofessionalpersonnel in area of expertise.
  • Mentors nursing personnel in all levels of clinical practice.
  • Participates in community programs, distribution of literature, asappropriate in area of expertise.
  • Communicates effectively with patients/families, physicians andancillary services at all levels to assure coordinated efficientpatient care.
  • Documents the plan of care, interventions and evaluations in aclear and concise manner for every patient visit in the patient’smedical record following approved hospital/UHA policies andcompliance standards for documentation.
  • Documents all interventions in the patient’s medical recordfollowing approved hospital/UHA policies and compliance standardsfor documentation.
  • Collaborates with other leaders and staff to effectively achievepatient outcome based improvements in care.
  • Provides leadership in the development and implantation of changesin the surgical urology practice that positively impact patientoutcomes.
  • Works collaboratively with medical and nursing staff to facilitatecreative problem solving and provide professional support.
  • Participates and initiates inter-disciplinary approaches to patientcare, program development and education. Serves as a professionalrole model.
  • Promotes and practices innovation of the expanded role in thedelivery of care to surgical urology patients and familiesthroughout the inpatient hospitalization, and the ambulatorysetting.
  • Participates and provides leadership for committees and taskforces.
  • Documents hours and submits billing reports, as appropriate.
  • Participates in the WVUM/UHA Performance Improvement Plan.
  • Applies information from Performance Improvement activities,changing practice as necessary, to improve care provided topatients.
  • Reviews and interprets surgical urology data from sources such aspatient registries, clinical and financial reports, length of stay,access to care, and cost per case reports and recommends/implementschanges as necessary.
  • Participates in clinical trials and research studies with thePrincipal Investigator.
  • Assists surgery Faculty, Administrators and Directors in thecollection of outcome measures.
  • Other duties/projects as assigned.
PHYSICAL REQUIREMENTS
  • Heavy/Hard Work: Work requires strength and/or stamina, lifting,moving, stooping, reaching, positioning of patients, standing,walking, and carrying of patients, materials and equipmentweighting 40+ pounds.
