The Advanced Practice Professional (APP) provides high-quality, patient-centered care as a member of the interdisciplinary healthcare team. Working collaboratively with physicians, nurses, and other healthcare professionals, the APP assesses, diagnoses, treats, and manages patients within their scope of practice and area of specialty. The APP is responsible for delivering evidence-based care, promoting positive patient outcomes, maintaining professional and regulatory standards, and supporting the clinical, educational, and operational goals of the department and organization.

POSITION SUMMARY : The Advanced Practice Professional (APP) has coordination and clinical skills required for the comprehensivemanagement of preoperative patients, (including biological, socialand psychological assessments). The APP coordinates the involvementof medical /surgical specialty teams necessary to develop acomprehensive optimization plan for the patient, reviews the planof care with the other members of the clinical team, participatesin directing that care, performs diagnostic/therapeutic proceduresand coordinates surgical planning (as applicable). The APPparticipates in the education of residents and medical/nursingstudents.

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER
EDUCATION, CERTIFICATION, AND/OR LICENSURE
  1. 1. Graduate of a CCNE accredited nursing program with Master’sDegree in Nursing .
  2. 2. National Certification by one of the recognized APRNCertification Centers .
  3. 3. Licensure or eligibility for licensure as a professionalRegistered Nurse and Advanced Practice Registered Nurse in WestVirginia.
  4. 4. American Heart Association BLS Certification required withadditional Advanced Life Support training at the discretion of the department.
EXPERIENCE
  1. 1. Preoperative and/or acute care experience preferred.
  2. 2. Eligible new graduates will be considered.
PREFERRED QUALIFICATIONS
EDUCATION, CERTIFICATION, AND/OR LICENSURE
  1. 1. ACNPs, CNSs and FNPs will be considered.

  3. 3. Certification by the National Commission for Certification ofPhysician Assistants required.
  4. 4. Must be licensed or eligible for licensure by the WV Board ofMedicine or WV Board of Osteopathic Medicine .
  5. 5. American Heart Association BLS Certification required withadditional Advanced Life Support training at the discretion of the department.

CORE DUTIES AND RESPONSIBILITIES

  1. 1. Consultation service ma nag ing comprehensive and individualizedcare for p reoperative patients and their families.
  2. 2. Completes optimization evaluations with pre-procedure/pre-surgical evaluations.
  3. 3. Completes integrated assessments.
  4. 4. With the collaboration of preoperative faculty or by followingestablished clinical Guidelines and Protocols, orders additionaltests or consults as deemed appropriate.
  5. 5. Performs and communicates results of diagnostic tests followingestablished clinical guidelines and scope of practice.
  6. 6. Consults and communicates with clinic, surgical and hospitalstaff regarding abnormal findings in the evaluations ofpatients.
  7. 7. Performs succinct comprehensive clinical evaluations anddocuments findings in medical record.
  8. 8. Facilitates patient optimization by coordinating appropriateservices and tests prior to surgery necessary for best outcomes (asapplicable) in the perioperative phase of patient care.
  9. 9. Develops a comprehensive individualized plan of care forpreoperative patients, which includes teaching, surgical andmedical planning and necessary referral to other members of thehealth care team as necessary.
  10. 10. Demonstrates expertise in caring for patients across the agecontinuum of their educational experience and certification.
  11. 11. Identifies unstable or complex preoperative patient caresituations and initiates appropriate intervention in the clinicand/or hospital setting.
  12. 12. Recognizes emergency situations and initiates effectiveemergency care.
  13. 13. Evaluates preoperative patients and serves as a n optimizationcoordinator for follow up procedures and tests in the ambulatorysetting (as applicable).
  14. 14. Assists in the collection of outcome measures (asapplicable).
  15. 15. Demonstrates critical thinking and diagnostic reasoning skillsin clinical decision making by the identification, evaluation andmanagement of the individual needs of patients/families.
  16. 16. Demonstrates the ability to utilize equipment in a safemanner.
  17. 17. Bases decisions made/actions taken on the relevant scientificprinciples, established standards of care, best outcomes and/orresearch findings.
  18. 18. Utilizes teaching/learning theory and appropriate teachingstrategies in the provision of education to nursing staff, hospitalstaff, students, patients, families and the community.
  19. 19. Promotes an environment that facilitates learning by respondingto appropriate patient/family preferences, priorities and readinessto learn.
  20. 20. Updates own knowledge base throughout the year.
  21. 21. Completes all identified clinical competencies.
  22. 22. Serves as a resource for staff caring for the preoperativepatient.
  23. 23. Provides educational programs to professional andparaprofessional personnel in area of expertise.
  24. 24. Participates in community programs, distribution of literature,as appropriate in area of expertise.
  25. 25. Documents the plan of care, interventions and evaluations in aclear and concise manner for every patient visit in the patient’smedical record following approved hospital/UHA policies andcompliance standards for documentation.
  26. 26. Collaborates with other leaders and staff to effectivelyachieve optimal patient outcome s based on improvements incare.
  27. 27. Provides leaders hip in the development and implemen tation ofchanges in the preoperative practice that positively impact patientoutcomes.
  28. 28. Works collaboratively with medical and nursing staff tofacilitate creative problem solving and provide professionalsupport.
  29. 29. Participates and initiates inter-disciplinary approaches topatient care, program development and education. Serves as aprofessional role model.
  30. 30. Promotes and practices innovation of an expanded role in thedelivery of care to preoperative patients and families throughoutthe inpatient hospitalization and the ambulatory setting.
  31. 31. Participates and provides leadership for committees and taskforces (as assigned) .
  32. 32. Documents hours and submits billing reports, asappropriate.
  33. 33. Reviews and interprets preoperative data from sources such aspatient registries, clinical and financial reports, length of stay,access to care, and cost per case reports and recommends/implementschanges as necessary.
  34. 34. Participates in clinical trials and research studies with thePrincipal Investigator.
  35. 35. Assists preoperative Faculty, Administrators and Directors inthe collection of outcome measures.
  36. 36. Other duties/projects as assigned.
PHYSICAL REQUIREMENTS
  1. 1. Heavy/Hard Work: Work requires strength and/or stamina, lifting,moving, stooping, reaching, positioning of patients, standing,walking, and carrying of patients, materials and equipmentweighting 40+ pounds.
