Job summary

The West Virginia University Health System invites applications for an Advanced Practice Professional with hematology/oncology expertise. The APP will perform comprehensive patient health evaluations, coordinate care with the clinical team, and manage discharge planning in infusion and ambulatory settings.

Collaboration with hem/onc faculty, teaching residents and nursing students, and adherence to state licensure and professional certification requirements are essential.

Qualifications

• Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing.
• National Certification by one of the recognized APRN Certification Centers.
• Licensure or eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
• American Heart Association BLS Certification required with additional Advanced Life Support training as determined by department.

Responsibilities

• Manages comprehensive and individualized care for patients and their families in infusion center and ambulatory setting.
• Completes daily rounds; conducts pre-treatment evaluations and participates in ambulatory clinics and inpatient care.
• Performs integrated assessments and coordinates tests/consults as appropriate.
• Orders tests or consults per guidelines and scope of practice.
• Communicates results of diagnostic tests and abnormal findings to care team.
• Assists with discharge planning and follow-up scheduling for outpatient care.
• Performs procedures following SOPs and scope of practice.
• Develops an individualized plan of care for hematology/oncology patients including teaching and referrals.
• Participates in education of residents and medical/nursing students.

POSITION SUMMARY

The Advance Practice Professional(APP) is an individual with clinical skills and medical skills,with an expertise in the care of Hematology/Oncology (hem/onc)patients. The APP completes comprehensive patient healthevaluations (including biological, social, and psychologicalassessments), considering their co-morbid conditions and overalldisposition; in collaboration with the appropriate clinical/facultyteam. The APP assesses and coordinates the educational andpsychosocial needs of the patient and caretakers. The APP developsand reviews the plan of care with the other members of the clinicalteam, participates in directing that care, and coordinatesdischarge planning/and or follow-up (as applicable). The APPparticipates in the education of residents and medical/nursingstudents and performs diagnostic procedures and therapeuticprocedures.

Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable. Suchservice may include but are not limited to.

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER
