The Advanced Practice Professional (APP) provides high-quality,patient-centered care as a member of the interdisciplinaryhealthcare team. Working collaboratively with physicians, nurses,and other healthcare professionals, the APP assesses, diagnoses,treats, and manages patients within their scope of practice andarea of specialty. The APP is responsible for deliveringevidence-based care, promoting positive patient outcomes,maintaining professional and regulatory standards, and supportingthe clinical, educational, and operational goals of the departmentand organization.

POSITION SUMMARY

The Advance Practice Professional(APP) is an individual with clinical skills and medical skills,with an expertise in the care of Hematology/Oncology (hem/onc)patients. The APP completes comprehensive patient healthevaluations (including biological, social, and psychologicalassessments), considering their co-morbid conditions and overalldisposition; in collaboration with the appropriate clinical/facultyteam. The APP assesses and coordinates the educational andpsychosocial needs of the patient and caretakers. The APP developsand reviews the plan of care with the other members of the clinicalteam, participates in directing that care, and coordinatesdischarge planning/and or follow-up (as applicable). The APPparticipates in the education of residents and medical/nursingstudents and performs diagnostic procedures and therapeuticprocedures.

Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable. Suchservice may include but are not limited to.

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

• 1. Graduate of a CCNE accredited nursing program with Master’s Degreein Nursing

MINIMUM QUALIFICATIONS FOR PHYSICIAN ASSISTANT

• 1. Bachelor’s Degree or Master’s degree as Physician Assistantrequired.
• 2. Graduate of an ARC-PA approved Physician Assistant (PA) trainingprogram required.
• 3. Certification by the National Commission for Certification ofPhysician Assistants required.
• 4. Must be licensed or eligible for licensure by the WV Board ofMedicine or WV Board of Osteopathic Medicine.
• 5. American Heart Association BLS Certification required withadditional Advanced Life Support training at the discretion ofthe department.

CORE DUTIES AND RESPONSIBILITIES

1. 1. Manages comprehensive and individualized care for patients andtheir families in the infusion center and in the ambulatorysetting.
2. 2. Completes daily rounds; consults and/or pre-treatment orpre-procedure/evaluations in the infusion center (as applicable).Participates in ambulatory clinics. Participates in inpatient careof patients.
3. 3. Completes integrated assessments.
4. 4. With the collaboration of hem/onc faculty or by followingestablished clinical Guidelines and Protocols, orders additionaltests or consults as deemed appropriate.
5. 5. Performs and communicates results of diagnostic tests followingestablished clinical guidelines and scope of practice.
6. 6. Consults and communicates with clinic and hospital staff regardingabnormal findings in the evaluations of patients.
7. 7. Performs succinct comprehensive clinical evaluations and documentsfindings in medical record.
8. 8. Facilitates patient discharge and/or follow-up byordering/coordinating appropriate services and tests prior todischarge from the infusion center necessary for outpatientfollow-up appointments in the clinic (as applicable).
9. 9. Performs necessary procedures following established clinicalguidelines and Protocols and scope of practice (SOP).
10. 10. Develops a comprehensive individualized plan of care for hem/oncpatients, which includes acute management, teaching, dischargeplanning and necessary referral to other members of the health careteam as necessary.
11. 11. Demonstrates expertise in caring for patients across the agecontinuum of their educational experience and certification.
12. 12. Participate in physician rounds, (as applicable).
13. 13. Identifies unstable or complex hem/onc patient care situations andfacilitates appropriate intervention in the infusion center, clinicand/or hospital setting.
14. 14. Recognizes emergency situations and initiates effective emergencycare.
15. 15. Evaluates hem/onc patients and directs follow up procedures andtests in the ambulatory setting (as applicable).
16. 16. Assists in the collection of outcome measures (asapplicable).
17. 17. Demonstrates critical thinking and diagnostic reasoning skills inclinical decision making by the identification, evaluation, andmanagement of the individual needs of patients/families.
18. 18. Demonstrates the ability to utilize equipment in a safemanner.
19. 19. Bases decisions made/actions taken on the relevant scientificprinciples, established standards of care and/or researchfindings.
20. 20. Utilizes teaching/learning theory and appropriate teachingstrategies in the provision of education to nursing staff, hospitalstaff, students, patients, families and the community.
21. 21. Promotes an environment that facilitates learning by responding toappropriate patient/family preferences, priorities, and readinessto learn.
22. 22. Evaluates the effectiveness of patient/family teaching andcollaborates with the nursing and clinical staff in the completionof education plan.

24. 24. Completes all identified clinical competencies.
25. 25. Serves as a resource person for staff caring for the hem/oncpatient.
26. 26. Provides educational programs to professional and paraprofessionalpersonnel in area of expertise.

28. 28. Participates in community programs, distribution of literature, asappropriate in area of expertise.
29. 29. Communicates effectively with patients/families, physician, andancillary services at all levels to assure coordinated efficientpatient care.
30. 30. Documents the plan of care, interventions, and evaluations in aclear and concise manner for every patient visit in the patient’smedical record following approved hospital/UHA policies andcompliance standards for documentation.
31. 31. Documents all interventions in the patient’s medical recordfollowing approved hospital/UHA policies and compliance standardsfor documentation.
32. 32. Collaborates with other leaders and staff to effectively achievepatient outcome-based improvements in care.

36. 36. Promotes and practices innovation of the expanded role in thedelivery of care to hem/onc patients and families in the infusioncenter, the inpatient hospitalization and the ambulatorysetting.

41. 41. Reviews and interprets hem/onc data from sources such as patientregistries, clinical and financial reports, length of stay, accessto care, and cost per case reports and recommends/implementschanges as necessary.
42. 42. Participates in clinical trials and research studies with thePrincipal Investigator.
43. 43. Assists Faculty, Administrators and Directors in the collection ofoutcome measures.
