SMG System Medical Group in West Virginia is seeking an Advanced Practice Professional (APN/NP) to provide coordinated, evidence-based care for behavioral medicine patients. You will work with medical and nursing staff to develop comprehensive care plans and participate in resident and student education.

The Advanced Practice Professional (APP) provides high-quality,patient-centered care as a member of the interdisciplinaryhealthcare team. Working collaboratively with physicians, nurses,and other healthcare professionals, the APP assesses, diagnoses,treats, and manages patients within their scope of practice andarea of specialty. The APP is responsible for deliveringevidence-based care, promoting positive patient outcomes,maintaining professional and regulatory standards, and supportingthe clinical, educational, and operational goals of the departmentand organization.

Additional Job Description:

POSITION SUMMARY : The Advanced Practice Professional (APP)has coordination and clinical skills required for the comprehensivemanagement of behavioral medicine patients, ( including biological,social and psychological assessments). The APP coordinatesinvolvement of medical/surgical specialty teams necessary todevelop a comprehensive plan for the patient, and reviews the planof care with the other members of the clinical team, participatesin directing that care, performs diagnostic/therapeutic procedures,and coordinates ongoing management (as applicable) . The APPparticipates in the education of residents and medical/nursingstudents.

Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable . Suchservice may include, but are not limited to .

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER :
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
  • Graduate of a CCNE accredited nursing program with Master’s Degreein Nursing .
  • National Certification by one of the recognized APRN CertificationCenters .
  • Licensure or eligibility for licensure as a professional RegisteredNurse and Advanced Practice Registered Nurse in WestVirginia.
  • BLS Certification required with additional Advanced Life Supporttraining at the discretion of the department.
EXPERIENCE:
  • Acute care or behavioral medicine experience preferred .
  • Eligible new graduates will be considered.
PREFERRED QUALIFICATIONS :
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
  • BLS certification required . Additional life support certificationat the discretion of the department
EXPERIENCE:
  • Acute care, surgical or behavioral medicine experience preferred,CAQ in Psychiatry preferred
  • Eligible new graduates will be considered.
PREFERRED QUALIFICATIONS :
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
  • Masters level education as Medical/Surgical (as applicable)
CORE DUTIES AND RESPONSIBILITIES:
  • Manages comprehensive and individualized care for behavioralmedicine patients and their families in the behavioral medicinehospitalization and/or ambulatory setting.
  • Completes daily rounds; consults and/or emergent/evaluations.
  • Completes integrated assessments.
  • With the collaboration of behavioral medicine faculty or byfollowing established clinical Guidelines and Protocols, ordersadditional tests or consults as deemed appropriate .
  • Performs and communicates results of diagnostic tests followingestablished clinical guidelines and scope of practice.
  • Consults and communicates with clinic and hospital staff regardingabnormal findings in the evaluations of patients.
  • Performs succinct comprehensive clinical evaluations and documentsfindings in medical record .
  • Facilitates patient discharge by coordinating appropriate servicesand tests prior to discharge necessary for outpatient follow-upappointments in the clinic (as applicable) .
  • Develops a comprehensive individualized plan of care for behavioralmedicine patients, which includes teaching, discharge planning andnecessary referral to other members of the health care team asnecessary.
  • Demonstrates expertise in caring for patients across the agecontinuum of their educational experience and certification.
  • Identifies unstable or complex behavioral medicine patient caresituations and facilitates appropriate intervention in the clinicand/or hospital setting.
  • Recognizes emergency situations and initiates effective emergencycare.
  • Evaluates behavioral medicine patients and serves as a coordinatorfor follow up procedures and tests in the ambulatory setting (asapplicable).
  • Assists in the collection of outcome measures (asapplicable).
  • Demonstrates critical thinking and diagnostic reasoning skills inclinical decision making by the identification, evaluation andmanagement of the individual needs of patients/families.
  • Demonstrates the ability to utilize equipment in a safemanner.
  • Bases decisions made/actions taken on the relevant scientificprinciples, established standards of care and/or researchfindings.
  • Utilizes teaching/learning theory and appropriate teachingstrategies in the provision of education to nursing staff, hospitalstaff, students, patients, families and the community.
  • Promotes an environment that facilitates learning by respondingtoappropriate patient/family preferences, priorities and readiness tolearn.
  • Updates own knowledge base throughout the year.
  • Completes all identified clinical competencies.
  • Serves as a resource person for staff caring for the behavioralmedicine patient.
  • Provides educational programs to professional and paraprofessionalpersonnel in area of expertise.
  • Participates in community programs, distribution of literature, asappropriate in area of expertise.
  • Documents the plan of care, interventions and evaluations in aclear and concise manner for every patient visit in the patient’smedical record following approved hospital/UHA policies andcompliance standards for documentation .
  • Collaborates with other leaders and staff to effectively achievepatient outcome based improvements in care.
  • Provides leadership in the development and implantation of changesin the behavioral medicine practice that positively impact patientoutcomes .
  • Works collaboratively with medical and nursing staff to facilitatecreative problem solving and provide professional support.
  • Participates and initiates inter-disciplinary approaches to patientcare, program development and education . Serves as a professionalrole model.
  • Promotes and practices innovation of the expanded role in thedelivery of care to behavioral medicine patients and familiesthroughout the inpatient hospitalization, and the ambulatorysetting.
  • Participates and provides leadership for committees and taskforces.
  • Documents hours and submits billing reports, as appropriate .
  • Reviews and interprets behavioral medicine data from sourcessuchas patient registries, clinical and financial reports, length ofstay, access to care, and cost per case reports and recommends/implements changes as necessary.
  • Participates in clinical trials and research studies with thePrincipal Investigator.
  • Assists behavioral medicine Faculty, Administrators and Directorsin the collection of outcome measures.
  • Other duties/projects as assigned.
