Job summary

CrescentCare in New Orleans seeks an Advanced Practice Registered Nurse to deliver comprehensive primary care within a collaborative team. You will evaluate patients, perform exams, diagnose, treat, and coordinate care with physicians and specialists in a community-focused setting.

Join a team dedicated to HIV elimination, HCV reduction, women's health, contraception education, and gender care while promoting wellness and preventive services for diverse communities in Greater New Orleans.

Qualifications

• Master’s degree in science with APN certification.
• Minimum 1 year experience in a clinical setting.
• HIV-focused care and LGB health experience preferred.
• Bilingual (Spanish/English) preferred.
• Louisiana RN license and APN certification required.
• DEA certificate and active NPI number required.
• Board certification required; BLS/ACLS as necessary.

Responsibilities

• Provide full range of core primary care services within collaborative agreement.
• Perform histories, physical exams, diagnostics, and treatment planning.
• Prescribe pharmacologic and non-pharmacologic therapies per guidelines.
• Document care in EMR and maintain informed consent as needed.
• Coordinate referrals and transitions between care settings.
