Job summary

Qualifications

Role Description

The Advanced Practice Nurse will provide comprehensive, patient-centered care in an on-site clinical setting in Leesburg, FL ( with light travel to The Villages and/or Bushnell, FL). In this full-time role, responsibilities include conducting patient assessments, diagnosing and managing acute and chronic conditions, ordering and interpreting diagnostic tests, and developing individualized treatment plans. The Advanced Practice Nurse will perform routine follow-ups, provide preventive care and health education, and coordinate with physicians, specialists, and other healthcare professionals to ensure continuity of care. Daily tasks also involve maintaining accurate clinical documentation, supporting quality improvement initiatives, and contributing to a welcoming, inclusive environment for patients of diverse backgrounds, including veterans. The role may include participation in community health efforts aligned with Premier Medical’s mission of expanding access to compassionate, high-quality care.

Qualifications

• Active Advanced Practice Registered Nurse (APRN) or Nurse Practitioner license in the state of Florida, with national board certification (e.g., FNP, AGNP, or equivalent).
• Demonstrated clinical skills in primary care, urgent care, and chronic disease management, including assessment, diagnosis, and evidence-based treatment.
• Strong patient communication, counseling, and education abilities, with a commitment to culturally competent and trauma-informed care.
• Experience collaborating within multidisciplinary teams, including physicians, nurses, pharmacists, and allied health professionals.
• Proficiency in using electronic health records (EHR) systems and maintaining accurate, timely documentation.
• Ability to manage a diverse caseload in a fast-paced clinic setting while maintaining attention to detail and patient safety.
• Prior experience in community health, family medicine, or internal medicine is highly beneficial; experience serving veteran populations is a plus.
• Graduate degree in Nursing (MSN or DNP) from an accredited institution; current BLS certification required, with ACLS preferred.
