Job summary

Atrius Health, part of the Optum family, seeks an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to join our Plymouth, MA team. You’ll provide longitudinal primary care, coordinate care, and collaborate with specialists to manage a panel of patients.

Active MA NP/PA license, national board cert, DEA, and BLS are required. Two+ years APC experience preferred. Compensation ranges from $109,500 to $164,000 plus benefits. Equal opportunity employer.

Qualifications

  • Graduate of an Accredited Nurse Practitioner or Physician Assistant Program.
  • Active, unrestricted Massachusetts NP or PA license.
  • National Board Certification.
  • Current active Mass DPH Controlled Substances Registration or ability to obtain post-hire.
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire.
  • Maintains BLS certification, professional certification as appropriate.

Responsibilities

  • Comprehensive, longitudinal clinical and administrative coordination of care for a panel of patients.
  • Examination, diagnosis and treatment in a primary care setting with care plan supervision and referrals.
  • Oversees monitoring and management of acute and chronic illnesses and adjusts treatment as needed.
  • Partners with patients and families to manage care plans.
  • Provides consultation regarding hospitalized patients in the panel.
  • Provides direction and support to staff to ensure departmental effectiveness and efficiency.

Position Highlights & Primary Responsibilities:

Comprehensive, longitudinal clinical and administrative coordination of all care for a specific panel of patients.

Performs all activities related to the examination, diagnoses and treatment of patients in a primary care setting, including physical assessment and treatment; supervision of care plan, to include consultations, referrals and communications with and to specialists, as appropriate

Oversees the monitoring and management of patients with acute and chronic illnesses. Serves as the care plan author and supervisor. Adjusts treatment regimens based on the patient’s response to treatment. Ensures that the on-going management of patients

Partners with patients and families to manage care plan

Provides consultation regarding hospitalized patients in the panel

Provides direction and support to staff to assure departmental effectiveness and efficiency

Required Qualifications:

Preferred Qualifications:

2+ years of APC experience

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
