Job summary

Grammar LLC is seeking an Advanced Practice Clinician (Nurse Practitioner/Physician Assistant) to support our mission-driven community health centers and mobile medicine programs. You will examine patients, evaluate symptoms, diagnose conditions, develop treatment plans, and administer medical care in collaboration with the Medical Director.

Qualifications

• Must hold California RN, NP, and NP board certification for NPs.
• CA PA license, PA certificate, and NCCPA board certification for PAs.
• 1-2 years clinical experience serving underserved populations.
• Current BLS and DEA certifications.
• Valid CA driver license and auto insurance.

Responsibilities

• Observe, assess, and document patient signs, symptoms, and responses to treatment; initiate referrals or emergency procedures.
• Examine patients using physical findings, lab results, and medical history to ascertain illnesses and prescribe treatments.
• Develop and institute treatment plans per protocol for acute and chronic medical problems with Medical Director consultation.
• Direct and oversee patient care services, including medication administration within licensure.
• Provide patient education, counseling, and disease prevention services.
• Maintain accurate patient medical records in accordance with agency and JCAHO standards.

• Observe,assess,and document patient signs,symptoms,and responses to treatment; initiate appropriate reporting,referrals,or emergency procedures.
• Examine patients using physical findings,lab results,and medical history to ascertain illnesses and prescribe treatments.
• Develop and institute appropriate treatment plans per protocol for acute and chronic medical problems with Medical Director consultation.
• Direct and oversee patient care services,including medication administration and therapeutic agents within the scope of licensure.
• Provide patient education,counseling,and disease prevention services.
• Maintain accurate patient medical records in accordance with agency and JCAHO standards.

Requirements:

• For Nurse Practitioners:Current California RN,NP,and NPF licenses,plus current Nurse Practitioner board certification.
• For Physician Assistants:California Physician Assistant License,PA Certificate,and NCCPA board certification.
• 1-2 years of clinical experience working with underserved,low-income,or homeless populations preferred.
• Current BLS and DEA certifications.
• Current and valid California driver's license and proof of personal auto insurance.

Ideal Candidate:

A dedicated Nurse Practitioner or Physician Assistant with experience serving low-income or homeless communities, possessing strong clinical judgment in managing acute and chronic conditions.Bilingual or multilingual skills (Spanish, Russian,Farsi,or Pashto) are a strong plus.
